Please keep your contact information current throughout the school year, especially if there is any change in phone numbers or addresses, so we can notify you as soon as possible if your child becomes ill, gets injured, or has any other emergency while at school.

It is also important to list several other emergency contacts in case we are unable to reach you for some reason.

All students in the Kirksville R-III School District must have a health form on file.

Immunization Requirements

Please send verification of any immunizations (vaccines) that your child receives to their school Health Services office, so we can keep your child's record up-to-date.

8th Grade and 12th Grade Immunization Requirements
Missouri now requires all students entering into the 8th grade to have one MCV (Meningococcal Vaccine) and one Tdap (tetanus, diphtheria and pertussis) prior to school starting. 

All students entering the 12th grade are required to have two doses of MCV unless the first dose is administered on or after the age of 16, then only one dose is required.  


Medications at School

If your child requires medication to be given at school; the Kirksville R-lll School District policy states:

A) It must be in the original prescription bottle with a current pharmacy label in place.

B) Children may not carry medication of any type to or from school, even if it's an over the counter medication.

C) A parent/guardian must bring the medication to the nurse and fill out a medication authorization form for it to be given.
